Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting
- Monitor and observe patient conditions, reporting any changes or concerns to healthcare professionals
- Administer medication and manage medical documentation following HIPAA guidelines
- Perform vital signs assessments including blood pressure, pulse, respiration, and temperature
- Support mobility using equipment like Hoyer lifts and assist with transfers safely
- Provide memory care for clients with Alzheimer's or dementia, ensuring safety and comfort
- Assist with medical procedures such as catheter care and hospice support as needed
- Prepare meals tailored to dietary restrictions and assist with feeding when necessary
- Perform light housekeeping tasks including laundry and cleaning to maintain a safe environment
- Document patient care activities accurately using EMR/EHR systems
- Support emotional well-being through companionship and engaging activities tailored to individual needs
Requirements
- Prior experience in home health, nursing homes, assisted living, or long-term care preferred
- Knowledge of medical terminology, medication administration, and patient care procedures
- Certification in CPR is required; additional certifications such as hospice care or dementia training are a plus
- Ability to work independently while following healthcare plans and protocols
- Strong communication skills for effective interaction with clients, families, and healthcare teams
- Experience working with people with developmental disabilities or special needs is desirable
- Familiarity with EMR/EHR systems for medical documentation is preferred
- Ability to perform physical tasks including lifting (Hoyer lift), standing for extended periods, and assisting with mobility needs
- Compassionate attitude with a focus on patient safety and dignity
- Valid driver’s license is advantageous for travel between client locations
