Position SummaryThe Social Worker supports individuals engaged in peer support services by providing assessments, care coordination, referrals, and recovery-focused planning. This role works collaboratively with Certified Peer Specialists and other providers to ensure individuals receive appropriate, ethical, and effective support services. The ideal candidate values peer-led care, recovery principles, and culturally responsive practices.This position does not provide psychotherapy but focuses on supportive services, engagement, resource coordination, and program support in alignment with Pennsylvania peer support guidelines.Key ResponsibilitiesConduct psychosocial assessments, screenings, and intake evaluations as needed to support peer services eligibility and care planning.Collaborate with Certified Peer Specialists to support person-centered, recovery-oriented service delivery.Assist individuals in identifying goals, strengths, needs, and community resources.Provide care coordination, referrals, and linkage to behavioral health, medical, housing, employment, and social services.Support crisis prevention and stabilization planning in collaboration with peers and supervisors.Ensure services align with trauma-informed care, recovery principles, and peer support best practices.Maintain accurate and timely documentation in the electronic health record (EHR) in compliance with organizational, state, and funding requirements.Educate individuals on their rights, confidentiality, and available supports.Participate in team meetings, supervision, training, and quality improvement activities.Maintain professional boundaries while supporting peer-led service delivery.Comply with all Pennsylvania Department of Human Services (DHS) regulations and organizational policies.QualificationsBachelor’s or Master’s degree in Social Work (BSW or MSW) required.Current Pennsylvania social work license (LSW or LCSW) preferred.Experience working in mental health, behavioral health, or community-based services.Strong understanding of recovery-oriented systems of care and peer support values.Experience working collaboratively with Certified Peer Specialists strongly preferred.Knowledge of trauma-informed care, cultural competency, and strengths-based approaches.Excellent communication, organizational, and interpersonal skills.Proficiency with electronic documentation systems.Ability to maintain confidentiality and ethical standards.Preferred QualificationsExperience in peer support or recovery-focused programs.Familiarity with Pennsylvania DHS behavioral health and peer support service models.Experience working with diverse populations, including individuals with serious mental illness and/or substance use challenges.BenefitsCompetitive compensationHealth, dental, and vision insurancePaid time off and holidaysProfessional development and training opportunitiesSupportive, mission-driven work environmentFlexible scheduling optionsAbout Peerstar:PeerStar is Pennsylvania's largest and most established peer support provider, operating exclusively in the peer support space since 2008.
