- Coordinate Resident Care: Collaborate with healthcare teams to develop and implement personalized care plans that meet the unique needs of each resident
- Assist with Daily Living Activities: Provide assistance with daily living activities such as bathing, dressing, and grooming to support residents' independence
- Monitor Resident Health: Conduct regular wellness checks and report any changes or concerns to the healthcare team
- Maintain Accurate Records: Document resident care plans, progress notes, and vital signs in accordance with company policies and regulations
- Communicate with Families: Keep families informed of their loved ones' progress and any changes or concerns
- Participate in Quality Improvement Initiatives: Contribute to quality improvement initiatives to ensure the highest level of care and comfort for residents

- Ability to lift 25 pounds and assist residents with transfers and mobility
- Strong communication and organizational skills
- Knowledge of senior care principles and practices
- Current CPR/First Aid certification
- Flexibility to work varied shifts, including weekends and holidays
