This role also includes documenting patient progress and collaborating with the interdisciplinary team to ensure the highest level of care.

Position Overview
The Physical Therapist (PT) is responsible for evaluating, planning, and delivering individualized treatment programs using established physical therapy practices. This role also includes documenting patient progress and collaborating with the interdisciplinary team to ensure the highest level of care.
Key Responsibilities
- Evaluate and treat residents with temporary or long-term disabilities
- Develop and implement individualized plans of care
- Deliver treatment to improve mobility, reduce pain, and restore function
- Educate patients, families, and caregivers on therapy plans and progress
- Monitor patient responses and adjust treatments as needed
- Accurately document treatments and patient progress in compliance with regulations
- SNF or long-term care experience preferred
- Strong knowledge of current therapy practices
- Experience with Net Health (or similar EMR) preferred
- Ability to work effectively with diverse patient populations
- Current Physical Therapist license in the State of New Mexico
- License must be active and in good standing
