Position Overview
As an Occupational Therapist (OT), you will evaluate, plan, and implement treatment for residents using evidence-based approaches to improve independence and quality of life. You’ll also play a key role in patient and family education, interdisciplinary collaboration, and documentation of care.
Key Responsibilities
- Evaluate residents to determine functional impairments and therapy needs
- Develop and implement individualized treatment plans focusing on ADLs and IADLs
- Monitor patient progress and adjust treatment as needed, collaborating with the interdisciplinary team
- Recommend adaptive equipment and home modifications to promote safety and independence
- Educate patients, families, and caregivers on techniques and strategies to support functional recovery
- Maintain thorough and timely documentation in accordance with state and federal guidelines
- Attend required meetings and actively participate in care planning
- Ensure compliance with all regulatory and facility policies
- Active Occupational Therapist license
- In good standing with all relevant licensing boards and agencies
- Experience in Skilled Nursing Facilities (SNF) or long-term care preferred
- Familiarity with Net Health documentation system is a plus
- Strong interpersonal skills and the ability to work with diverse patient populations
