Position Overview:
We are seeking a detail-oriented and clinically proficient Registered Nurse to serve as an MDS Coordinator Assistant. In this role, you will support the coordination and completion of the Resident Assessment Instrument (RAI) process, ensuring the accurate and timely completion of Minimum Data Set (MDS) assessments and individualized care plans. This position plays a critical role in driving clinical quality outcomes, maintaining regulatory compliance, and optimizing reimbursement under PDPM and other applicable payment models.
Key Responsibilities:
- Coordinate and assist in the completion of MDS assessments, Care Area Assessments (CAAs), and comprehensive care plans in accordance with CMS guidelines and state regulatory requirements.
- Accurately document residents’ clinical status, services provided, and care needs through detailed chart review and interdisciplinary collaboration.
- Partner with the interdisciplinary team—including nursing, therapy, dietary, and social services—to ensure timely and accurate assessments and care planning.
- Monitor changes in resident condition and ensure appropriate and timely updates to MDS assessments and care plans.
- Transmit MDS data electronically to CMS and ensure successful submission with validation of error-free reports.
- Audit resident medical records to ensure documentation supports MDS coding accuracy and regulatory compliance.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives, including quality measure monitoring and performance improvement activities.
- Provide education and guidance to nursing and clinical staff regarding documentation standards and MDS processes.
- Maintain current knowledge of RAI manual updates, CMS regulations, and state requirements to ensure ongoing compliance.
- Support Medicare and Medicaid reimbursement processes by ensuring accurate coding aligned with PDPM and case mix methodologies.
- Attend and contribute to interdisciplinary care plan meetings, supporting the development of individualized resident-centered care plans.
- Assist with floor nursing responsibilities and participate in on-call coverage as needed to support operational and clinical demands.
- Graduate of an accredited nursing program with a current, active Registered Nurse (RN) license in good standing.
- Minimum of one (1) to two (2) years of experience in MDS coordination within a skilled nursing facility.
- Strong working knowledge of MDS 3.0, the RAI process, PDPM reimbursement methodology, and applicable federal and state regulations.
- Proficiency in electronic medical record systems and MDS software (e.g., PointClickCare).
- Demonstrated ability to manage multiple priorities, meet regulatory deadlines, and work independently.
