Key Responsibilities
- Lead and manage all social service programs within the facility.
- Conduct resident/family interviews and complete assessments and care plans.
- Provide information on Medicare/Medicaid and other assistance programs.
- Coordinate with external agencies when needed.
- Ensure accurate and timely MDS documentation.
- Hire, train, supervise, and evaluate department staff.
- Participate in surveys, audits, and quality improvement efforts.
- Bachelor’s degree in Social Work or related human services field required.
- MSW preferred.
- Minimum 2 years of social work experience in a healthcare setting.
- Supervisory experience preferred.
- Strong communication, decision-making, and interpersonal skills.
