The TCU Nurse is responsible for providing high-quality nursing care to patients transitioning from hospital to home or long-term care, ensuring a smooth and safe recovery process.

Qualifications
- Current RN or LPN license in the State of RI
- Graduate of an accredited nursing program
- Previous experience in skilled nursing, rehab, or acute care preferred

Responsibilities
- Provide direct patient care in accordance with individualized care plans
