The responsibilities of our Clinical Navigators include:
Patient Education & Support
- Act as a resource for patients and families by explaining available services and answering questions about care options.
- Schedule and meet with patients and families and provide clear, compassionate education on the benefits of Home Health, Hospice, and Outpatient Therapy when appropriate.
- Empower patients and families to make informed decisions about their care plans.
Clinical Review & Needs Identification
- Review patient charts and new referrals to identify potential needs for additional support or services.
- Recommend educational conversations based on clinical appropriateness and patient goals.
Collaboration & Coordination
- Work closely with homecare, hospice, and outpatient therapy clinicians and leadership to ensure seamless communication and care transitions.
- Participate in interdisciplinary team meetings to support patient-centered planning.
Process Development & Improvement
- Help design and refine workflows that make it easier for patients to access information and resources about available services.
- Ensure processes prioritize patient choice and quality of care.
Compliance & Documentation
- Maintain accurate records of patient interactions and educational efforts in accordance with regulatory and company standards.
Data Tracking & Reporting
- Track and report all referral conversion data across service lines.
- Maintain detailed logs of outreach efforts, patient interactions, and outcomes.
- Provide regular reports to leadership on conversion metrics and trends.
- All other duties as assigned
Qualifications:
- Current clinical license in the state of Ohio (RN preferred).
- Strong understanding of hospice eligibility and other care options, with the ability to explain them clearly and compassionately.
- Minimum 2 years of clinical experience in home health, hospice, or related field.
- Excellent communication and interpersonal skills, with a patient-first mindset.
- Highly organized and detail-oriented, with the ability to develop supportive processes.
- Advanced spreadsheet and data management skills for tracking, follow-up, and documentation.
Job Conditions:
- This is a desk position that involves sitting, standing, stooping, and walking as well as an inordinate amount of telephone communication.
- This position requires minimal lifting.
- One must be able to adequately hear on the telephone, with no more than an amplifier and be able to communicate both verbally and in writing, in English.
