Salary: Homecare Clinical NavigatorTerritory: 986 Tibbetts Wick Rd, Girard, OH 44420Business Hours:Monday - Friday, 8:00 AM to 4:30 PMPatriot is veteranownedand family operatedWhy Patriot?Patriot At Home is a placewhere you candirectlymake animpactinyour community while alsothrivingpersonally and professionally!We recognize family as the cornerstone of ourcompany,andwantyou tobe part of a team that values your contributions and well-being.We value our families and community:Paid time offHardship Program-program tosupport staffduring challenging timesPatriot Cares-nonprofit forcommunityoutreachHow we support you professionally:This position is paid at a competitive hourly rateMedical/Dental/Vision Insurance/Supplemental insurance(beginsimmediatelyupon hire)Company 401K with4%company matchingThe responsibilities of our Clinical Navigators include:Patient Education & SupportAct as a resource for patients and families by explaining available services and answering questions about care options.Schedule and meet with patients and families and provide clear, compassionate education on the benefits of Home Health, Hospice, and Outpatient Therapy whenappropriate.Empower patients and families to make informed decisions about their care plans.Clinical Review & Needs IdentificationReview patient charts and new referrals toidentifypotential needs foradditionalsupport or services.Recommend educational conversations based on clinical appropriateness and patient goals.Collaboration & CoordinationWork closely with homecare, hospice, and outpatient therapy clinicians and leadership to ensure seamless communication and care transitions.Participate in interdisciplinary team meetings to support patient-centered planning.Process Development & ImprovementHelp design and refine workflows that make it easier for patients to access information and resources about available services.Ensure processes prioritize patient choice and quality of care.Compliance & DocumentationMaintainaccuraterecords of patient interactions and educational effortsin accordance withregulatory and company standards.Data Tracking & ReportingTrack and report all referral conversion data across service lines.Maintain detailed logs of outreach efforts, patient interactions, and outcomes.Provide regular reportstoleadership on conversion metrics and trends.All other duties as assignedQualifications:Current clinical license in the state of Ohio (RN preferred).Strong understanding of hospice eligibility and other care options, with the ability to explain them clearly and compassionately.Minimum 2 years of clinical experience in home health, hospice, or related field.Excellent communication and interpersonal skills, with a patient-first mindset.Highly organized and detail-oriented, with the ability to develop supportive processes.Advanced spreadsheet and data management skillsfor tracking, follow-up, and documentation.Job Conditions:This is a desk position that involves sitting, standing, stooping, and walking as well as an inordinate amount of telephone communication.This position requires minimal lifting.One must be able to adequately hear on the telephone, with no more than an amplifier and be able to communicate both verbally and in writing, in English.Patriot at Home is an equal opportunity employer
