Position Summary:
We are seeking an experienced Licensed Vocational Nurse (LVN) to join our growing team on a Pay-Per-Visit basis. The LVN will provide skilled nursing care to patients in their homes, ensuring accurate documentation and timely communication with the office team.
Responsibilities:
- Perform skilled nursing visits and follow-up visits
- Monitor patient condition and report changes to supervising RN/physician
- Assist with implementation of individualized care plans
- Provide patient and family education on medications and disease processes
- Complete documentation using Axxess software and EVV (Electronic Visit Verification) for all visits
- Communicate effectively with physicians and interdisciplinary team
- Ensure compliance with all state, federal, and agency regulations
Requirements:
- Active Texas LVN license (in good standing)
- Home Health experience preferred
- Axxess software
- OASIS knowledge is a plus
- Reliable transportation, valid driver’s license, and auto insurance
- Strong organizational and communication skills
- Dependable, compassionate, and professional attitude
- Bilingual in English and Spanish is a plus (not required)
