What are the responsibilities and job description for the In Home Caregiver position at Patient Centered Services Home Health?
Patient Centered Services is currently seeking an experienced and compassionate PCA Personal Care Attendant/Caregiver to join our home health team in the Dallas/Fort Worth area.
Requirements:
- At least 6 months experience as a attendant/caregiver
- Reliable phone for EVV clock in/out (HHA Exchange)
- Reliable transportation and valid driver’s license
- Ability to travel throughout the Fort Worth and surrounding area
- Professional, dependable, and patient-centered attitudeResponsibilities:
- Assist patients with personal care and activities of daily living
- Complete documentation accurately in HHA Exchange
- Communicate effectively with PHC Supervisor
- Deliver compassionate care in patients’ home
Position Details:

We currently have two clients within five minutes apart. If you enjoy assisting the elder and disable you want to be a part of our team. We are looking for someone who is organized, experienced in caregiver/attendant in the home health work environment, and comfortable working independently while providing excellent patient care.
