POSITION SUMMARY

Pathways to Life is expanding its ACT team and adding psychiatric care provider hours to match a growing census. This role works in partnership with, and under the supervision of, the team psychiatrist, sharing the psychiatric care provider function. It is a fully integrated team role: the provider attends daily team meetings and sees members where they live rather than only in an office.

Important: under NC Medicaid Clinical Coverage Policy 8A-1, the roles and responsibilities of the ACT psychiatric care provider cannot be met through telemedicine or telepsychiatry. This position requires in-person service delivery.

ESSENTIAL DUTIES

  • Assess and treat members’ psychiatric symptoms and monitor medication response and side effects, with most members seen within four to six weeks of their last appointment and many contacts occurring in the community

  • Collaborate with the team leader in sharing clinical responsibility for monitoring member treatment

  • Work with the team’s nurses to develop and implement medication administration policies and to oversee members’ medical care, including regular health screenings

  • Educate team members on psychiatric and non-psychiatric medications, side effects, and related health conditions

  • Provide diagnostic and medication education to members using a shared decision-making approach

  • Participate in daily team meetings and treatment planning meetings in proportion to scheduled hours

  • Provide brief formal or informal therapy

  • Share psychiatric back-up coverage after hours and on weekends on a rotating basis

MINIMUM QUALIFICATIONS

  • Nurse Practitioner: current NC licensure meeting the approval and practice parameters in 21 NCAC 36.0800, with at least three years of full-time experience treating individuals with serious and persistent mental illness

  • Physician Assistant: current NC licensure meeting the requirements of 21 NCAC 32S.0200, with at least three years of full-time experience treating individuals with serious and persistent mental illness

  • Current DEA registration

  • Valid North Carolina driver’s license, reliable transportation, and auto insurance meeting agency minimums

  • Ability to pass a criminal background check, drug screen, healthcare sanctions screening, and health screening including TB

PREFERRED QUALIFICATIONS

  • Prior experience on an ACT team or in another community-based psychiatric setting

  • Experience with long-acting injectable antipsychotics and clozapine monitoring

  • Familiarity with TMACT fidelity standards

REQUIRED TRAINING

The following must be completed within the first 120 calendar days of hire:

  • DHHS-approved High-Fidelity ACT training

  • Crisis Response training

  • Brief Motivational Interviewing

  • Person-Centered Thinking

These requirements are waived where the candidate provides written documentation of successful completion within the previous 12 months. Applicants transferring from another North Carolina ACT team are encouraged to submit training transcripts with their application.
