Health Homes Care Coordinator

Job Overview
Come join the team at Passages Family Support as a Health Homes Care Coordinator. The Care Coordinator will work with clients, their providers, both at Passages and in the community, to assess, facilitate, plan and coordinate an integrated delivery of care across the continuum, including behavioral health, physical health, and long-term care for clients with high need potential. Passages staff work to ensure that clients progress toward desired outcomes with quality care that is clinically appropriate and cost-effective based on the severity of illness. Please note: this is not a home health position. The position requires at a minimum a bachelor's degree.

Required qualifications:

· Bachelor's or Master's Degree in a social science, psychology, nursing or social work or related field

· 2-3 years’ experience as a care coordination/case management in the health care field - either physical health or behavioral health.

Benefits include a competitive benefits package with employer-paid health insurance premium, a generous sign on bonus, and a 35 hour work week paid at 40 hours (Passages closes at noon every Friday).
