Expectations:
Responsible for coordination of care to meet identified needs of the patient/family. Supervises the management of patient care utilizing critical thinking and nursing process, discharge planning, resource identification, linking and scheduling patients with required services, monitoring care delivery, advocating for the patient, care planning and evaluating outcomes. Coordinates patient care across the health care continuum, and assists other team members with meeting individual patient needs. This position may require flexibility of hours.
EDUCATION:
Bachelor’s degree (BSN) required. Associate Degree in Nursing (ADN) will be considered if enrolled in a BSN program with degree completion within 2 years of date of hire or date of transfer to an RN position.
EXPERIENCE/TRAINING:
Minimum of one (1) year of relevant experience in acute care area of expertise/equivalent service area required, preferably advanced Med/Surg experience with telemetry and completion of an approved Critical Care Core Curriculum program.
Experience with electronic documentation/EHR systems required.
LICENSES/CERTIFICATIONS:
Required:
Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN)
Current Basic Life Support Provider (BLS), Advanced Cardiovascular Life Support Provider (ACLS) issued by American Heart Association (AHA)
Current National Institutes of Health Stroke Scale (NIHSS) certification within 30 days of hire/transfer
Preferred:
Current Critical Care registered Nurse (CCRN) or Progressive Care Certified Nurse (PCCN)
SKILLS:
Basic computer skills.
Care of the patient with Pulmonary monitoring and/or support, including ventilators.
Care of the patient requiring vasoactive drip medications.
Care of the Thoracic patient including management of chest tubes.
Care of the post-operative stable Open-Heart patient, including transthoracic epicardial pacing.

Pay Range: The estimated base rate for this position is $47.88 - $74.20.Additional individual compensation may be available for this role through differentials, extra shift incentives, bonuses, etc. Base pay is only a portion of the total rewards package, and a comprehensive benefits program is available for qualifying positions.
