MDS Coordinator Job Duties & Requirements:
- Included in this role will be medical records
- Have current RN license in the state of Ohio
- Completing and updating MDS forms on all patients in the care facility
- Performing clinical assessments to gather information on residents
- Developing care plans for patients to coordinate available services
- Submitting updates and paperwork to maintain compliance
- Participating in the admission and discharge processes
- Conducting and attending meetings with other care team members
- Investigating complaints or issues from residents and their families
- Educating patients and their families
- Additional duties & requirements as given by job description, regional MDS coordinator, regional operator, and administrator
