Care Coordinator, RN or LPN
Full Time
1st Shift (7AM - 7:30PM)
Includes holiday and weekend rotation.
The Care Coordinator is key personnel responsible for managing admissions for the facility. Assisting the clinical management team by overseeing treatment plans, collaborating with other team members to ensure all residents are cared for by completing assessments, treatments, monitoring and developing resident goals and communicating progress. The Care Coordinator is dedicated to providing support to staff in order to provide quality care for the residents.

Qualifications:
Minimum Requirements:
- Must be a licensed LPN;
- Must have any necessary required certifications where needed;
- Must have high level of computer skills and PCC knowledge;
- Demonstrate the ability and willingness to work harmoniously with all Department Heads, personnel, ODH staff, and residents;
- Must be able to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public;
- Must have patience, cheerful disposition, and enthusiasm handling difficult situations;
- Must have leadership skills and excellent communication skills;
- Must possess the ability to make independent decisions when circumstances warrant such action;
- Must be willing to seek out new methods and principles and be willing to incorporate them into existing policies and procedures;
- Must be willing to be open to change and meet the requests of Administration and Nursing;
- Must communicate to the highest level to Administration, peers and staff.
