Job Summary
We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. Responsibilities
- Conduct comprehensive patient assessments to evaluate physical function, mobility, and pain levels using detailed physiology and anatomy knowledge.
- Develop and implement tailored care plans that incorporate evidence-based physiotherapy practices such as trigger point therapy and orthopedics.
- Deliver hands-on therapy sessions focused on improving strength, flexibility, and functional independence for diverse patient populations including pediatrics, geriatrics, and those recovering from injury or surgery.
- Coordinate discharge planning by collaborating with multidisciplinary teams to ensure seamless transition from hospital to home or community care settings.
- Utilize electronic medical records (EMR) systems efficiently to document patient progress, treatment plans, and clinical notes accurately.
- Educate patients and their families on injury prevention, home exercise programs, and self-care strategies to promote ongoing recovery outside clinical environments.
- Stay current with advancements in physiotherapy techniques and maintain compliance with healthcare standards through continuous professional development.
Requirements
- Valid licensure as a licensed Physical Therapist in the relevant jurisdiction with current registration.
- Proven experience in acute care hospital settings, outpatient clinics, or home health environments demonstrating strong patient care skills.
- In-depth understanding of medical terminology, physiology, and human anatomy essential for accurate assessment and treatment planning.
- Experience working with diverse populations including pediatrics, geriatrics, orthopedics, and patients requiring inpatient or outpatient rehabilitation services.
- Familiarity with EMR systems for efficient documentation and patient record management.
- Knowledge of discharge planning processes and ability to coordinate care across inpatient, outpatient, home & community care settings.
