Key Responsibilities:
- The Registered Nurse (Hospice) at Parentis Hospice and Health is responsible for providing compassionate and skilled nursing care to patients in the hospice setting. This role involves coordinating with interdisciplinary teams to ensure that patients receive comprehensive end-of-life care that respects their dignity and comfort while supporting their families through the process.
Key Responsibilities:
- Conduct comprehensive assessments of patients to identify their physical, emotional, and psychosocial needs.
- Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and interdisciplinary team members.
- Administer medications and treatments as prescribed, monitoring patients for side effects and efficacy.
- Provide education and support to patients and their families regarding the disease process, symptom management, and end-of-life care options.
- Coordinate with physicians, social workers, and other healthcare professionals to ensure seamless delivery of care.
- Maintain accurate and timely documentation of patient care activities in compliance with regulatory and organizational standards.
- Participate in case conferences, team meetings, and quality improvement initiatives to enhance patient care and service delivery.
- Offer emotional support and guidance to patients and families, addressing concerns and facilitating effective communication.
- Ensure adherence to hospice care policies, procedures, and ethical standards.
- Engage in continuous professional development to stay current with best practices in hospice and palliative care.
Required Education:

Required Skills and Abilities:
- Bachelor of Science in Nursing (BSN) from an accredited institution
- Valid Registered Nurse (RN) license in the state of practice
Required Experience:
- Minimum of two years of clinical nursing experience, preferably in hospice or palliative care settings
- Experience in managing patient care plans and coordinating with interdisciplinary teams
- Proven track record of providing compassionate care to patients with terminal illnesses
