Service Coordinator

We are currently seeking an organized and devoted Service Coordinator to join our team!

JOB SPECIFICATIONS

Responsibilities:

• Support residents and families with day-to-day needs, concerns, and resources.
• Welcome new residents and assist with their transition into the community.
• Coordinate services, referrals, assessments, and community resources based on residents’ changing needs.
• Work closely with the interdisciplinary team, outside agencies, and service providers.
• Advocate for residents and provide support with sensitive care-related matters, including Advance Directives and POLST when appropriate.
• Maintain accurate, confidential resident records in accordance with HIPAA.
• Assist with resident programs, including fall prevention, meals, parking, and other community initiatives.
• Prepare the monthly resident calendar and newsletter.
• Coordinate educational, health, wellness, and social programs, including at least three original programs each month.
• Attend monthly staff meetings and required training.
• Perform other duties as needed.

Qualifications:

• Strong communication, customer service, organization, and problem-solving skills.
• Ability to work independently and as part of an interdisciplinary team.
• Knowledge of community resources and government programs.
• Comfortable working with residents and families in sensitive or stressful situations.
• Computer proficiency, including Word, Excel, and Outlook.
• Understanding of confidentiality and HIPAA requirements.
• Spanish speaking required.

Job Type: Full-time

Pay: $25.00 - $27.00 per hour
