PRN Licensed Vocational Nurse will provide direct patient care to home bound patients.

- Provides wound care, disease process education, medication administration, and other clinical duties as prescribed by NP's, PA's, and Physicians.
- Carrys out the clinical plans of care to assist patients maintain their independence.
- Coordinates with the interdisciplinary clinical team regarding patient changes in condition and proactively communicates patient's clinical needs.
- Provides education to patients and families on proper home health care procedures and strategies.
- Advocates for appropriate patient care and recommends strategies to help improve the patient's quality of life.
- Current Texas LVN License.
- Current Certification in BLS/CPR
- Valid Texas Driver's License and Automobile Insurance
- Knowledge of WellSky EMR software is a plus
