Speech Language Pathologist

Responsibilities and Duties:

• Assess patient needs and develop individualized treatment plans based on physician orders and interdisciplinary team input.
• Provide direct speech therapy services to patients in their homes or assisted living facilities within a defined geographic area.
• Ensure timely documentation and compliance with all Paradigm HomeCare policies and procedures.
• Collaborate with physicians, nurses, and the care team to support optimal patient outcomes.
  
  

Qualifications & Skills:

• Master's Degree in SpeechLanguage Pathology from an accredited university
• Active SpeechLanguage Pathologist license in the State of Texas
• Current CPR certification
• Valid Texas Driver's License and automobile insurance
• Minimum 3 years of experience as a Speech Language Pathologist, with at least 1 year of recent home health experience (within the last three years)
• Knowledge of WellSky EMR software desired
