- Assess patient needs and develop individualized treatment plans based on physician orders and interdisciplinary team input.
- Provide direct speech therapy services to patients in their homes or assisted living facilities within a defined geographic area.
- Ensure timely documentation and compliance with all Paradigm HomeCare policies and procedures.
- Collaborate with physicians, nurses, and the care team to support optimal patient outcomes.
- Master's Degree in SpeechLanguage Pathology from an accredited university
- Active SpeechLanguage Pathologist license in the State of Texas
- Current CPR certification
- Valid Texas Driver's License and automobile insurance
- Minimum 3 years of experience as a Speech Language Pathologist, with at least 1 year of recent home health experience (within the last three years)
- Knowledge of WellSky EMR software desired
- SelfMotivated and able to work independently while meeting required deadlines
- Clear communicator, both written and oral
- Respectful, professional, and courteous in all client, team, and public interactions
- Collaborative team player with a patientfirst mindset
