Position Details:
- Full TimeChaplain for Mishawaka, Indiana and surrounding communities
Experience:
- One year Hospice experience is preferred, but not required
Job Functions:
- Performs accurate and comprehensive spiritual assessments within a 5 day window from the start of care date.
- Ensures that changes in patients’ clinical status are communicated to other healthcare team members and HHA office staff verbally and in writing in a timely manner.
- Coordinates care planning and delivery with the physician, patient/family, other healthcare team members and HHA staff to facilitate optimal patient outcomes.
- Acts as liaison between Hospice and the spiritual community.
- Responds to the mental, spiritual, social and emotional needs of terminally ill patients and their families.
- Provides nondenominational spiritual counseling per patient and/or family request.
- Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
- Communicates appropriately and clearly to physicians, staff, Clinical Nurse Manager and administrative team.
- Assists family in understanding the grief process.
- Participates in the bereavement program as requested including facilitating grief support groups, participating in the celebration of life programs, leading funeral services, providing bereavement counseling via phone or in person, sending bereavement mailing, and notifying hospice staff of funeral services.
- Gives referrals to family for community grief support groups.
- Allows patient and family to voice their concerns, fears and feelings.
- Responsive to patient’s desires and condition.
- Addresses and supports cultural practices, as long as such practices do not harm others.
- Supports a culture of safety and quality.
- May perform specific religious duties related to the faith in which he/she is ordained, i.e., administration of last rites, baptism.
- Understands Hospice philosophy and issues of death and dying.
- Complies with HHAs policies regarding documentation timeliness.
- Adheres to all applicable laws, regulations and standards as indicated by professional licensure at local, state and federal levels.
- Assists the physician and other Hospice team members to understand the spiritual, emotional and social factors related to the patient’s health problems so that each patient’s plan of care fosters spiritual well-being and human dignity.
- Treats patients and their families with respect and dignity.
- Participates in the on-call rotation
Credentials:
- Master's Degree in Ministry, Counseling or Social Work required
- Current driver's license
- Must have reliable transportation and insurance
