Licensed Vocational Nurse (LVN) - Home Health

What You'll Do:

• Provide skilled nursing care to patients in their homes according to physician orders.
• Monitor vital signs and report changes to the care team.
• Educate patients and families on medications, procedures, and home care.
• Collaborate with an interdisciplinary team to ensure patients receive holistic care.
• Perform additional duties as needed to support patient care and agency operations.

Employment Type: Full Time

Area Coverage: San Francisco area (travel required to patient homes)

What We Offer:

• Competitive, bi-weekly: Up to $44.00 per hour
• FULL IRS Mileage Reimbursement: of $0.725 per mile

Requirements:

• Active Vocational Nurse license in the State of California
• Active BLS/ACLS certification
• Valid Driver’s License and auto insurance
• Up-to-date immunization records (COVID-19, TB test, Flu, Hep B, MMR, TDaP, Varicella)
