What You'll Do
- Provide acute and longitudinal medical care to patients in skilled nursing and rehabilitation facilities.
- Perform comprehensive patient assessments, including history, physical examination, and diagnostic evaluation.
- Diagnose and manage acute and chronic medical conditions.
- Develop individualized treatment plans and prescribe medications within scope of practice.
- Collaborate with nursing staff, therapists, specialists, families, and facility teams to coordinate care.
- Perform procedures appropriate to training and scope of practice.
- Utilize evidence-based guidelines to provide high-quality, compassionate care.
- Maintain accurate and timely electronic documentation.
- Participate in interdisciplinary care planning to optimize patient outcomes..
Qualifications
- Active Washington State ARNP or Physician Assistant license.
- DEA license
- Strong clinical assessment and decision-making skills.
- Excellent communication and teamwork abilities.
- Ability to work independently within a collaborative care environment.
- Experience in geriatrics, skilled nursing, internal medicine, family practice, hospital medicine, or post-acute care.
- New graduates with a strong interest in skilled nursing and geriatric medicine are encouraged to apply.
Pay: $130,000.00 - $160,000.00 per year
