What are the responsibilities and job description for the Palliative Care Nurse Practitioner position at PacWest Healthcare, PLLC?
We are seeking a compassionate and experienced Nurse Practitioner (ARNP) to provide palliative care services to patients residing in skilled nursing facilities, assisted living communities, and adult family homes throughout the Spokane area.
This role focuses on improving quality of life for patients with serious illness through symptom management, goals-of-care discussions, and collaborative care with facility staff, families, and healthcare providers.
Responsibilities
- Provide comprehensive palliative care assessments and management
- Manage complex medical conditions and symptoms including pain, dyspnea, nausea, and anxiety
- Conduct goals-of-care conversations and advance care planning
- Collaborate with facility staff, caregivers, and interdisciplinary teams
- Document visits and care plans in the electronic health record
- Coordinate care with primary care providers and specialists
- Provide education and support to patients, families, and care teams
Qualifications
- Active Washington State ARNP license
- Palliative Care Certification (highly preferred)
- DEA license
- Experience in hospital, skilled nursing, or long-term care settings preferred
- Strong communication and clinical decision-making skills
- Ability to work independently and collaboratively
