What are the responsibilities and job description for the Chaplain position at Horizon Hospice & Palliative Care?
PURPOSE:
To provide spiritual and emotional care to terminally ill patients and their families as well as grief support to the family after the death of the patient. Ability to work with people of various religious faiths and cultures.
RESPONSIBILITES:
1. Perform spiritual assessment of Hospice Patients
2. Respond to spiritual and emotional needs of Patients and their families
3. Provide nondenominational spiritual counseling
4. Provide Grief support
5. Chart all patient and family interactions
6. Work in tandem with interdisciplinary team.
7. Provide emotional and spiritual support to staff
8. Conduct or assist in funeral/memorial services as needed.
WORKING CONDITIONS AND HAZARDS:
Work includes driving personal vehicle to patient’s home and visiting or attending patients in various degrees of cleanliness and physical comfort. Varying types of weather and road conditions are encountered. Employees are exposed to persons in physical distress including pain and disfigurement. Physical contact with patients is possible and may include exposure to body fluids and odors. Contact may place employee in “high-risk” category for exposure to blood borne pathogens.
QUALIFICATIONS:
Must be an Ordained Minister
Master’s degree in related field
Minimum three years’ experience in spiritual care.
Reliable transportation with appropriate license and insurance coverage
Computer knowledge and experience.
Salary : $28 - $30
