RN Home Care Generalist

Job Summary:

As an RN - Home Care Generalist, you will provide vital, structured support across our community-based services. In this versatile role, you will assist our Home Care RNs in the field (mileage paid) while also providing coverage for our in-house adult day center nurses as needed. Your daily responsibilities are well-supported by our collaborative team and include delivering skilled and acute visits, wound care, medication administration, and medication packing. Additionally, you will conduct nursing assessments, perform wellness visits, provide health education, and communicate closely with participants and their families. By managing detailed electronic documentation, assisting with CNA supervision, and supporting vaccination clinics, you will play a key role in ensuring seamless continuity of care.

At PACE-RI, we value work-life balance. This position offers a full-time Monday through Friday schedule from 8:00 AM to 4:30 PM, with a Saturday rotation required only once every 5 to 6 weeks. • Driver's License (Required)
