Role
As an RN Care Manager at PACE North, you’ll be more than a nurse - you’ll be a decision maker, advocate, and trusted partner in care. In this role, you will:
- Serve as a core member of the interdisciplinary team, shaping care plans and ensuring participants’ needs are met.
- Collaborate with staff, participants, and families to build strong, supportive relationships.
- Conduct initial and periodic health assessments.
- Monitor and coordinate supplies, equipment, and services.
- Visit participant homes and participate in the on-call rotation.
- Lead care transitions, including hospital-to-home discharge planning.
- Provide education, advocacy, and leadership that empowers participants to live their best lives.
- Demonstrates the ability to assess current health status, anticipate future care needs, and establish proactive, comprehensive plans of care that evolve as participants' conditions change.
- Applies sound clinical judgment and appropriate risk tolerance to support participants in remaining in their homes, including through advanced illness and end of life, while balancing participant choice, caregiver capacity, and regulatory requirements

What We’re Looking For
- Graduate of an accredited nursing program (Associate or Bachelor of Science in Nursing)
- Current, valid RN license in the State of Michigan
- Valid driver’s license
- Strong critical thinking skills, with the ability to thrive in dynamic, evolving situations
