Position Summary:
The Intake Service Coordinator assists participants in accessing waiver services, MA State Plan services, and other medical, social, and educational supports, regardless of funding source. This role combines administrative coordination with extensive fieldwork, ensuring participants receive person-centered care tailored to their individual needs and goals.
Primary Responsibilities:
- Conduct person-centered assessments and level of care evaluations at least annually.
- Develop and maintain individualized service plans, reviewing and updating them quarterly or as needed.
- Coordinate and facilitate access to approved and authorized services across various agencies and providers.
- Maintain regular contact with participants, including at least quarterly phone calls and field visits (2–3 days per week).
- Evaluate whether participants’ needs, goals, and outcomes are being met, making necessary adjustments to service plans.
- Document all interactions, assessments, and coordination activities within 24 hours of completion.
- Identify and assess all possible funding sources before submitting requests to waiver programs.
- Provide high-quality, compassionate service to participants and families, ensuring a person-centered and outcome-driven approach.
- Attend company meetings and training sessions in person as required.
- Education: Bachelor’s degree in Social Work, Psychology, or a related field; OR at least 3 years of experience in the social work field.
- Experience: Prior service coordination experience preferred.
- Skills:
- Proficiency in Microsoft Office, Teams, and online databases.
- Strong communication and organizational skills.
- Ability to manage a rotating caseload and travel frequently.
- Excellent documentation and time management abilities.
- Licenses/Certifications: Valid Driver’s License required.
Competency Requirements:
- Strong interpersonal and problem-solving abilities.
- Knowledge of community resources and waiver program structures.
- Ability to work independently while maintaining collaboration with interdisciplinary teams.
- Empathy, professionalism, and a commitment to participant advocacy.
- Adaptability to hybrid and field-based work environments.
- Schedule: Monday–Friday, 8:30 AM–5:00 PM.
- Work Setting: 20% remote/home office, 80% field visits within a 2–3 hour travel radius.
- Requires flexibility for occasional additional visits depending on participant status.
