We are seeking a Home Health Case Manager to oversee and coordinate all aspects of patient care in the home health setting. The Case Manager will be responsible for developing individualized care plans, conducting in-home assessments, collaborating with interdisciplinary team members, and ensuring compliance with regulatory standards.
Key Responsibilities

Manage a caseload of home health patients.

Conduct initial and ongoing assessments to determine the patient’s needs.

Develop, implement, and update individualized care plans.

Monitor patient progress and revise care plans as needed.

Educate patients and families on disease process, medications, and care.

Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
