Position Summary
The Medical Social Worker (MSW) provides professional social services to patients and families in their homes. This position assists patients in understanding and coping with social, emotional, and financial factors related to illness, recovery, and overall well-being. The MSW works closely with the interdisciplinary team to develop comprehensive care plans that support optimal patient outcomes in the home health setting.
Essential Duties and Responsibilities

Assess the psychosocial status of patients and families to identify emotional, social, and environmental needs related to the patient’s condition.

Develop and implement individualized social service care plans in collaboration with the interdisciplinary team.

Provide counseling and emotional support to patients and families to assist in coping with illness, disability, or end-of-life issues.

Educate patients and families about community resources and assist in accessing financial assistance, transportation, home care equipment, and other support services.

Coordinate referrals to appropriate community agencies or facilities when necessary.

Participate in case conferences, team meetings, and discharge planning to ensure continuity of care.

Maintain accurate and timely documentation in accordance with agency policies and regulatory requirements.

Advocate for patients’ rights and ensure services are delivered in a manner that preserves dignity and promotes independence.

Provide crisis intervention and grief counseling as needed.

Perform other related duties as assigned.
Qualifications
Education:

Master’s degree in Social Work (MSW) from an accredited school of social work required.
Licensure/Certification:

Current state licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) as required by state regulations.

Valid driver’s license and reliable transportation required.
Experience:

Minimum of one (1) year of experience in a healthcare or home health setting preferred.

Experience with community resources, counseling, and case management strongly desired.

Ability to work independently while maintaining collaborative relationships with the care team.
