What are the responsibilities and job description for the Certified Nursing Assistant position at Ozark Physical Medicine Center for Operations?
We are looking for a reliable CNA to care for individuals in a professional and considerate manner. You will work under the supervision of a registered nurse or doctor. CNA professionals must be first and foremost qualified and compassionate. The ideal candidate will be experienced in providing diverse basic care to those who need while remaining patient and calm. This position will also include working in medical records in an office setting. It is a demanding job but also very rewarding.
Responsibilities:
- Perform personal care (bathing, dressing, toileting, feeding, etc) according to the plan of care while providing safe, competent care to the patient.
- Document care provided according to agency policy.
- Report any deviations from the Plan of Care to the Case Manager (or designee if not available)
- Use Durable Medical Equipment safely
- Gives simple emotional and psychological support to the patient and other members of the household and establishes a relationship with patient and family which transmit trust and confidentiality.
- Performs routine housekeeping tasks as related to a safe and comfortable environment for the patient, as instructed by the RN of therapist.
- Take vital signs when on Hospice Aide Plan of Care
- Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences
- Confirms on a weekly basis, the scheduling of visits so that other necessary visits by staff members can be coordinated.
- Works with personnel of other community agencies involved in the patient's care as directed by the RN or therapist.
- Provide proper care and observation of patient’s skin to prevent breakdown of tissue over bony prominence. Reports any changes/areas of concern to Case Manager
- Assist with ambulation, transfers, range of motion if documented in the plan of care
- Provide respite for patient’s family/caregiver when on-site, as appropriate
- Participate in QAPI activities
- Participate in meetings, as appropriate and attend in-service as required by agency policy
- Assisting the department with medical records.
Job conditions:
- The ability to access patients’ homes which may not be routinely wheelchair accessible is required
- Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.
- Physical activities will include, walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients
