Location: Denver, Colorado
Schedule: Full-time, Monday–Friday with rotating on-call

Your Role

As an RN Case Manager at Ovation Hospice, you’ll be a vital part of a compassionate interdisciplinary team. You will:

• Manage a caseload of hospice patients in homes, assisted living communities, and facilities across the Denver metro area
• Provide personalized, expert care with empathy and respect, including pain and symptom management
• Assess patients and develop individualized plans of care with the interdisciplinary team
• Build meaningful connections with patients and families, and educate them through the end-of-life journey
• Coordinate with physicians, facility staff, and the care team for seamless care
• Provide direction and leadership to hospice aides and care staff
• Complete timely, accurate documentation in accordance with Ovation policies and Colorado and Medicare standards
• Participate in the on-call rotation

Qualifications

• Unrestricted RN license valid in Colorado (or multistate compact license)
• Hospice, home health, or med-surg experience preferred
• Compassionate, caring nature with strong interpersonal and communication skills
• Comfortable with computers and electronic medical records
• Current CPR/BLS certification
• Valid driver’s license, auto insurance, and reliable transportation

Pay: $95,000.00 - $105,000.00 per year
