Position Summary
Assist participants with activities of daily living (ADLs), including toileting, transferring, grooming, dressing, ambulation, and assistance with meals.

Assist participants with activities of daily living (ADLs), including toileting, transferring, grooming, dressing, ambulation, and assistance with meals.
- Provide respectful and compassionate care tailored to individual needs and preferences.
- Monitor and document participants’ conditions and report changes to program supervisor or designated Adult Day Services Leadership.
- Escort participants safely within the program area, including to and from the bathroom, multipurpose rooms and entrances.
- Encourages participants and assists in supporting participation in all ADS programming.
- Promotes individualized interaction and care according to the ADS Person-Centered Care, Philosophy Model and the ‘Willingness to learn’ new model of care based on Thomas Kitwood’s Detractors of Personhood
- Support daily activity programming by helping participants engage in recreational, educational, and social activities.
- Assist with serving meals and snacks, including set-up, clean-up, and assisting participants as needed.
- Maintain cleanliness and organization in program areas, ensuring a safe and hygienic environment.
- Follow all infection control protocols, safety guidelines, and confidentiality policies (HIPAA compliance).
- Foster positive relationships with participants, families, and team members.
- Participate in staff meetings and required training sessions.
- Perform other duties as assigned by the Adult Day Services leadership.
- Current Wisconsin C.N.A. certification
- High school diploma or equivalent
- Excellent customer service skills and ability to work in a team environment
