We are seeking motivated, compassionate, and clinically driven Occupational Therapists who are passionate about working with adult and geriatric populations and value autonomy, flexibility, collaboration, and professional growth.
Position Summary
The Occupational Therapist will evaluate and treat adult and geriatric patients within the community setting, focusing on safety, independence, cognition, functional mobility, activities of daily living (ADLs), instrumental activities of daily living (IADLs), and overall quality of life.
Therapists work closely with patients, caregivers, physicians, and interdisciplinary rehabilitation team members to develop individualized plans of care that support safe and meaningful functional outcomes.
Key Responsibilities
- Perform comprehensive occupational therapy evaluations and reassessments
- Assess home environments for safety concerns, environmental barriers, and fall risks
- Address underlying impairments impacting ADLs and IADLs
- Perform cognitive, perceptual, and visual assessments
- Develop and implement individualized treatment plans
- Recommend and train patients in the use of adaptive equipment and compensatory strategies
- Educate patients, caregivers, and family members on home programs and safety strategies
- Collaborate with physicians, caregivers, and interdisciplinary rehabilitation team members
- Complete timely and accurate electronic documentation in compliance with regulatory and company standards
Qualifications
- Master’s Degree or Doctorate in Occupational Therapy from an accredited program
- Active New York Occupational Therapy License required
- 1 year of experience working with older adults preferred

- Geriatric rehabilitation
- Cognitive rehabilitation
- Fall prevention and home safety assessments
- Functional mobility and transfer training
- Adaptive equipment training
- Neurological rehabilitation
- Home-based rehabilitation
- Strong clinical reasoning, communication, and interpersonal skills
- Ability to work independently while remaining collaborative within a team environment
- Home care, home health, outpatient geriatrics, or community-based rehabilitation experience preferred
Compensation & Benefits
- Competitive salary: $90,000 – $104,500 annually
