What are the responsibilities and job description for the Mental Health Case Manager (Community Navigator) position at Ottawa County?

Please Note: Ottawa County Community Mental Health is currently transitioning to an independent authority. As of January 1, 2027, this position will be a part of the Ottawa County Community Mental Health Authority. 

Under the supervision of the Director of Access and Connections, screens service requests for Community Mental Health services, connects individuals to resources, supports, and services within the community, provides referrals and assistance with navigating the behavioral health system, and coordinates care with community agencies. Services are provided face to face and over the phone. Services are provided at the CMH office and other locations, as necessary. 

This is a full-time, benefited position working out of the James Street Complex located at 12265 James Street, Holland, MI. Typical working hours are Monday - Friday, 8:00 a.m. to 5:00 p.m. 
The essential functions of this position include, but are not limited to, the following:
  1. Responds to service requests for assistance. 
  2. Provides follow-up contact to individuals who have had recent contact with the emergency department or who have been discharged from inpatient care, checking their well-being and connecting them to mental health services and resources. 
  3. Processes service and referral requests for mental health services from community providers.
  4. Assists callers with getting connected with community resources and services.
  5. Provides information to callers about Community Mental Health services and community treatment services.
  6. Facilitates access to services by assisting with scheduling appointments, finding transportation options, and help problem solve obstacles to treatment. 
  7. Provides case management as needed.
  8. Conducts face to face and phone interviews. 
  9. Conducts other duties as assigned.
Bachelor’s degree in a human services discipline is required. Two years of relevant experience in the behavioral health field is preferred. Knowledge of and experience with different cultural and ethnic groups within Ottawa County is strongly preferred. Lived experiences with mental illness, developmental disabilities, or substance use disorders are valued. Bilingual (Spanish) with written and verbal medical translation skills preferred.
Required Knowledge and Skills:
  1. Excellent organizational and time management skills.
  2. Excellent customer service skills.
  3. Good working knowledge of community resources.
  4. Ability to assess risk of harm.
  5. Good working knowledge of the principles and practices of medical record documentation and medical records management. Ability to provide timely clinical notes. 
  6. Understands and can apply a collaborative approach to both intra-agency and interagency relationships.
Physical Requirements:
Must be able to perform essential job functions with or without reasonable accommodations, including, but not limited to, visual and/or audio logical appliances, and devices to increase mobility.

Working Conditions:
Work is performed in an office/clinical environment.
