Mental Health Inpatuent Unit-Per Diem

BROAD FUNCTION:

Provides clinical social work services to individuals within an inpatient psychiatric setting, including psychosocial assessment, discharge planning, individual and group interventions, and coordination of care. Participates actively in interdisciplinary treatment team meetings to develop and implement patient-centered care plans. Maintains timely and accurate documentation in accordance with regulatory and organizational standards. May provide clinical supervision to students or less experienced staff as assigned.

MAJOR RESPONSIBILITIES:

  • Provide counseling for individuals and groups. Communicate with families pertaining to clients’ admission and discharge
  • Obtain social history on every client admitted, as well as other mandatory assessments.
  • Document daily progress in notes, Social Summaries, discharge planning and service coordinating. Review Bill of Rights and Health Care Proxy with each client.
  • Provide input into treatment plans and obtain releases of information to obtain collateral involvement.
  • Provide services as an individual therapist, including assessment of treatment needs, treatment planning, and coordination of services.
  • Lead comprehensive discharge planning for patients with complex psychosocial, medical, and behavioral health needs, coordinating closely with interdisciplinary teams, families, and external partners to ensure safe and appropriate transitions across the continuum of care.
  • Facilitate timely and individualized discharge plans by assessing post-acute needs, arranging community-based services (e.g., home health, skilled nursing, behavioral health follow-up), securing durable medical equipment, and addressing barriers such as housing, transportation, or insurance limitations.
  • Provide clinical services in accordance with managed care regulations when indicated. Obtain authorizations for services as necessary.
  • Maintain accurate and timely documentation and prepare periodic reports as required.
  • Establish and maintain effective relationships with division staff, other service providers, and the community at large.
  • Coordinate clients’ plans of treatment with other service providers as needed. Refer for additional/specialized services when indicated as necessary.
  • Maintain current professional knowledge of therapeutic concepts and techniques. Attend seminars, workshops and conferences when appropriate.
  • Participate in in-service education and competency-based activities as indicated.
  • Comply with state and federal government rules and regulations regarding health, safety and equal opportunity.

QUALIFICATIONS:

  • Master’s degree in Social Work, Mental Health Counseling, or a closely related field is required.
  • Must hold one of the following New York State licenses:
    • Licensed Master Social Worker (LMSW)
    • Licensed Clinical Social Worker (LCSW)
    • Mental Health Counselor (MHC) (Limited Permit)
    • Licensed Mental Health Counselor (LMHC)
  • Minimum of one (1) year of experience working with adults with serious mental illness is preferred.
  • Strong clinical assessment, crisis intervention, and discharge planning skills.
  • Knowledge of community mental health services, housing resources, care coordination systems, and relevant entitlement programs.
  • Able to work collaboratively as part of a multidisciplinary team in a fast-paced environment.
  • Experience with electronic medical records and documentation standards in behavioral health settings is a plus.

Pay Range: $31.50-$36.50/hr

Employee salary is based on commensurate experience and other qualifications.
