Summary of Role
The Nurse Navigator will provide support to the total joint replacement services of TOSC. This role will be involved with scheduling and providing patient education, ensuring compliance related to CMS Patient-Reported Outcomes (PROs) data gathering and reporting as well as follow-up with patients post-operatively up to 1 year.
Key Responsibilities
- Maintain processes which accurately and adequately schedule and track patient progress throughout the total joint replacement experience.
- Monitor OR schedule 1 month to 2 weeks out from the current date to ensure all total joint patients are contacted and have filled out HOOS and KOOS data, as well as being scheduled for the joint replacement class.
- Develop a high level of comfort with teaching the Road to Recovery class and develop back-up personnel to teach the class if needed.
- Assume responsibility for communication with the TOSC leadership regarding issues that arise.
- Monitor changes in CMS requirements and modify the program to ensure compliance.
- Work with Social Climb to ensure HOOS and KOOS data are gathered, tracked, and reported with the utmost accuracy.
- Maintain open, professional, and positive communication
- Use multi-disciplinary communication to maintain and improve patient care
- Utilize proper chain of command, maintain emotional control and diplomacy
- Communicate facts to all stakeholders to avoid misunderstandings and undue stress
- Maintain patient, employee, and company confidentiality
- Assist with patient inquiries and ensure prompt accurate replies.
- Maintain and send patient statistical quality data to ACHC.
- Provide reports to physicians and QAPI committee regarding PROs.
- Work with surgeons and their teams to ensure StreaMD is up to date with current and appropriate information.
- Effectively communicate with the multidisciplinary team (including Anesthesia, PAT and Surgeon AAs)
Education:
- Graduate of an accredited school of nursing
Certification and Licensure:
- BLS, ACLS, and PALS
- State of WI nursing license
Experience:
- 1 year of pre/post TOSC experience
- Strong technology skills including EMR and Microsoft Office Suite (Word, Excel, PowerPoint)

Physical Demands—
- Works in a clean, well lit, air-conditioned environment
- Occasional stress while performing multiple required tasks efficiently at once
- Possible exposure to infection, contagious disease, needle puncture, injury from sharp instruments, and hazardous gases and compounds.
- Ability to work in a fast-paced, rapidly changing environment
- Willing and able to travel between sites, work flexible days and hours as needed, including evenings
- Use office and medical equipment; must have good visual and hearing acuity
Based on the OSHA guidelines for protection against occupational exposure to Hepatitis and HIV, this position is described as Category II. This means that this position may have limited exposure to blood and body fluids. Policies are in place for Universal Precautions identifying risks and appropriate barrier protection.
