Role Description & Expectations
SLS Caregiver/Direct Support Professionals engage in a variety of daily living activities with clients, including housekeeping, meal preparation, making and keeping important appointments, personal hygiene, recreation, and other activities of daily living (ADLs) not listed. SLS Caregiver/Direct Support Professionals are expected to fulfill the following duties and expectations:
- Assist and support developmentally and/or physically disabled adults with Activities of Daily Living (ADLs) in the home and out in the community. Such ADLs include shopping, cooking, transportation, accompaniment to appointments, bathing, grooming, toileting, housekeeping, and other tasks not listed.
- Follow housekeeping instructions to help keep the home safe and sanitary for individuals.
- Administer medication per the individuals physician-assigned medication schedule, and to monitor the individual for side effects or other medication-related incidents.
- Complete a post-shift report that describes what occurred during the shift and which duties were performed.
- Fulfill your legal and ethical obligations as a mandatory reporter, including reporting ANY incidents of actual or suspected abuse and neglect of individuals.
- Work with the individual for the entire duration of your shift, and report any obstacles, delays, or other incidents affecting your ability to work with the individual during your shift ASAP.
- Communicate with individuals, their circle of support, legal representatives, Optimum Care SLS II personnel, and the Agency Director as appropriate.
- Adhere to the NADSP Code of Ethics at all times, pertaining especially to client dignity, privacy, and independence in decision-making.
- Attend required trainings and meetings related to the individuals care.
Qualifications
The following are required for this role:
- A valid drivers license.
- A vehicle with current insurance and registration.
- CPR/First Aid certification.
- Negative TB test result from within the past year, or willingness to obtain prior to start date.
- Minimum 1 year of caregiving experienceprior experience in behavioral health or nursing is also highly preferred.
- Ability to pass FBI/DOJ LiveScan background check and DMV records check.
- Schedule that permits working irregular hours, such as weekends, holidays, and/or overnights.
- Knowledge of best practices in providing care to vulnerable populations.

- Establish and maintain long-term working relationships with clients, coworkers, and community members.
- Accurately and diligently follow written and verbal instructions.
- Communicate verbally and in writing with clients, their circles of support, office and caregiving, and other stakeholders in the clients care.
- Nurture a pleasant, homelike atmosphere in the clients residence that aligns with the clients personal preferences.
