JOB SUMMARY:  Responsible to provide leadership, direction, support, and supervision to employees assisting people supported in the Acquired Brain Injury (ABI) & Day Habilitation (DH) program. Responsible for ensuring creative, innovative and meaningful daily programming occurs as well as the individualized day habilitation ISP, and POC goals are being met. Work with staff to make the community integration initiative priority for those who strive are on a pathway toward competitive employment. Will be an active participate as a member of the Management Team.

 

RESPONSIBILITIES:

Agency Mission/Quality Services

  • Understand and perform all job functions based on the mission and philosophical principles of Opportunity Works.
  • Adhere to the Ethical Code of Conduct for all employees and represent the agency in a positive image.
  • Ensure the health, safety, well-being, and human rights of all people supported by following all agency policies & procedures and accreditation & licensing standards.
  • Create a positive, team-oriented, holistic, and compassionate work environment that focuses on quality service delivery and program development.
  • Adhere to the agency timelines/schedule regarding attendance and punctuality.
  • Participation at all mandatory trainings, scheduled trainings and utilization of training information to improve job performance.
  • Act as a role model for people supported by demonstrating a professional manner in all aspects of job performance, including but not limited to proper dress attire.
  • Promote and model Positive Behavior Supports.

Management, Leadership and Documentation

  • Train, direct, and monitor employees to adhere to agency policies & procedures, accreditation & licensing standards and Ethical Code of Conduct.
  • Demonstrate good problem solving, conflict resolution skills, teamwork and provide leadership to employees regarding the same.
  • Demonstrate a continuous quality improvement philosophy, train and support employees around agency mission and philosophy and program development.
  • Identify potential management problems and offer solutions to address such issues.
  • Advocate for program needs in a way that is respectful of other programs’ and agency-wide needs.
  • Demonstrate good organizational and time management skills.
  • Strive for professional growth and development through various opportunities.
  • Keep current on best practices in management and in the human services field to allow for the highest level of practices within the agency.
  • Complete annual performance evaluations for employees.
  • Provide quarterly supervisions to employees that focus on support, professional development and progress in their job performance.
  • Assist all staff in achieving their professional development goals.
  • Work in conjunction with the Director of Human Resources regarding the implementation of Personnel Policies including hiring/termination and progressive discipline process of personnel.
  • Work with Management team in providing orientation to new staff.
  • Responsible for staffing solutions ie. Call-outs, vacations, sick calls (please note these absences may require your attention during nonscheduled work hours) 

Clinical Support

  • Direct and supervise staff in their quest for providing assistance in developing problem-solving and decision-making skills for those we support.
  • Provide leadership in identifying interfering behavior and to ensure the implementation of behavioral interventions, protocols, and advising to people supported.
  • Identify clinical needs of people supported and make recommendations for strategies or support plan development and/or referral to appropriate agency consultants.
  • Inform the Division Director and Director of Behavior Services of all serious clinical issues regarding people supported including recommendations and follow up plans to address issues.
  • Consult with Division Director and Director of Behavior Services when specific behavior trainings are necessary. 
  • Work in conjunction with Director of Behavior Services in planning, facilitating and follow-thru (minutes, implementation of plans, staff training, etc…) of weekly clinical meetings. 
  • Ensure all employees have the ability to implement behavior plans as written.
  • Complete all incident reports required for each program and send to appropriate personnel.
  • Promote and model a culture of Positive Behavior Supports

Oversight of Staff Responsibilities

  • Oversee that staff provide people supported with annual mandated trainings and/or other pertinent trainings as determined.
  • Ensure that the staff provides respectful feedback and adjustment of techniques and individualized support to people to assist them to achieve and maintain independence at program.
  • Oversee that staff assist people supported in the development of their goals and interests based on their stated vision, values, and preferences.
  • Monitor that staff thoroughly complete documentation in a timely manner.
  • Participate in the interdisciplinary team process for people supported and ensure that quality input and information regarding the service needs and outcomes for people is provided to the team.
  • Provide leadership and direction to staff regarding the implementation of support plan goals and strategies.
  • Facilitate regularly scheduled team meetings with staff.
  • Train and support employees to work with people supported on developing good work habits, work skills, and interpersonal skills to allow for success in their program.
  • Ensure that all information regarding people supported is maintained with the highest level of confidentiality, based on policies and procedures, within a manner based on dignity and respect to people supported and regulations regarding people’s protected health information (HIPAA).
  • Complete required documentation that is accurate and completed within established timelines.
  •  Ensure that all emergency information regarding people supported, including their Emergency Fact Sheets, contain current and accurate information at all times.
  • Together with the Division Director, oversee that assessments (from internal and external clinicians and OW assessments) are completed in an accurate and objective manner based on the skills and skill level of people supported and to ensure quality and achievable goal recommendations.
  • Responsible for ensuring that all quarterly, monthly, and bi-annual notes are completed for the ABI and Day Hab Programs.
  • Complete and hand in all necessary paperwork according to deadlines.

Advocacy/Communication

  • Support people served to foster their growth and independence, to make choices, and to exercise their human rights people according to their individual communication styles and respectful of their specific support needs.
  • Advocate for people supported regarding their personal well-being, goals, and interests.
  • Communicate with the department Coordinators to ensure a continuum of service delivery.
  • Communicate all major incidents, accidents, and situations to the Division Director and to direct staff to do the same in the absence of the Program Manager.
  • Maintain good communication and coordinated efforts with residential providers, DDS Service Coordinators, MRC, ABI, families, and all other outside people.

Occupational Exposure Tasks/Procedures

The following is a list of procedures and tasks performed at Opportunity Works in which occupational exposure may occur:

 

  • Assistance with personal care including toileting, changing people who are incontinent of urine and/or feces, or cleaning people who have vomited.  May also come in contact with semen/vaginal secretions or saliva.
  • Behavioral interventions that may involve a person becoming incontinent of urine and/or feces or situations in which interventions may involve saliva or vomit.
  • Assessment of illness of a person that may involve body fluids.
  • Blood sugar testing (task normally performed by licensed nurses)
  • Administering injections (task performed by licensed nurses)

General

  • Adhere to all personnel policies, especially regarding the importance of good attendance and punctuality.
  • Follow through on all assigned tasks.
  • Work cooperatively with all team members and agency personnel.
  • Supervise people supported at all times including their arrival and departure.
  • Assist people with their personal care needs.
  • Ensure a clean and safe environment and to report any health or safety concerns promptly to the appropriate person based on policies and procedures.
  • Assist Transportation by driving or monitoring with morning and afternoon runs.
  • Complete other duties as assigned. This may include assisting in other departments and may be outside your normal tasks.

Qualifications: 

  • Bachelor’s Degree in health related field preferred
    • 3 years of relevant health care experience
    • 2 of those years must be spent in supervisory roll
  • In lieu of Bachelor’s Degree
    • 6 years of relevant health care experience
    • 3 of those years must be spent in a supervisory roll
  • Must have excellent verbal, written communication skills, computer and organizational skills.
  • Valid driver’s license and the ability to drive a van preferred
  • Crisis intervention training (i.e. PABC) is favorable
  • CPR, First Aid and MAP certification preferred.
