What are the responsibilities and job description for the Social Care Navigator position at Open Door Mission?

SUMMARY OF POSITION: The Social Care Navigator plays a vital role in helping individuals—particularly Medicaid enrollees—address health-related social needs (HRSNs) that affect their overall well-being. This position bridges the gap between healthcare and community-based resources, empowering individuals to access services such as housing, food, transportation, and behavioral health supports.

DUTIES AND RESPONSIBILITIES:

· Conduct comprehensive HRSN screenings and eligibility assessments to identify unmet needs among Medicaid members and other community participants.

· Connect individuals to community-based services such as housing assistance, food support, transportation programs, and behavioral health resources.

· Facilitate “closed-loop” referrals by confirming services have been accessed and needs are being met.

· Conduct proactive outreach to members within assigned geographic areas, ensuring consent is obtained and trust is built.

· Maintain consistent contact to support engagement and follow-through.

· Coordinate with internal teams (including care managers, dietitians, and clinicians) as well as community-based organizations to ensure continuity of care and holistic support.

· Accurately document all member interactions in electronic platforms and track progress using data management systems

· Supervise shelter and common areas, providing support, encouragement and accountability for services provided to ODM clients.

· Provides crisis intervention, de-escalation and debriefing in situations of conflict, stress and trauma.

· Ensure all policies and procedures are followed.

· Maintain current and complete case notes for each program participant in a timely manner. Develop proficiency in case management software.

· Attend team and staff meetings, as assigned.

· Communicate with the Guest Care Committee about client disciplinary issues.

· Demonstrates a courteous and Christ-like manner with internal and external partners.

· Agrees with and carries out responsibilities in accordance with the Mission’s policies that are included in the Open Door Mission Directives, Code of Conduct, Statement of Faith, and Mission Statement.

· Committed to serving the poor and homeless.

· Presents a history of stable and responsible employment.

· Possesses excellent skills in oral and written English communications.

· Possesses the ability to work effectively with others in a positive, cooperative manner.

· Willingly signs a confidentiality agreement.

· Must be in agreement with the goals and objectives of The Open Door Mission.

· Social Services Knowledge: Understanding of social determinants of health, health equity, and community resource networks.

· Communication and Engagement: Exceptional interpersonal and motivational interviewing skills to build rapport and inspire self-advocacy.

· Cultural Competency: Demonstrated ability to provide trauma-informed, culturally responsive care with empathy and respect.

· Technical Proficiency: Comfortable using data systems and technology tools for case documentation and workflow management.

· Problem Solving: Skilled at identifying barriers and developing creative solutions to connect individuals with needed resources.

· Attention to Detail: Consistent accuracy in documentation, follow-up, and data entry

WORKING CONDITIONS/PHYSICAL FACTORS:

Works in office areas as well as throughout the facility. Sits, stands, bends, lifts, and moves intermittently during working hours. Interacts with staff, visitors, etc., under all conditions/circumstances. May be required to work extended periods of time at a computer requiring repetitive motion. Infrequent light physical effort required.

· Analytical with attention to detail.

· Relational in all types of business cultures and with fellow team members.

· Organized in time management and in record keeping.

· Problem solver and solution oriented.

EDUCATION/EXPERIENCE: Educational requirements are currently under review. Possible qualifications may include:

· Bachelor’s degree in Social Work, Human Services, Public Health, or a related field; or

· Credentialed Recovery Peer Advocate (CRPA) certification; or

· Equivalent combination of education, certification, and relevant field experience.

Pay: $24.00 - $26.00 per hour
