Open Door Mission is seeking a Case Manager to support our ESSHI supportive housing program, serving individuals transitioning from homelessness and facing challenges such as mental health conditions, substance use, or domestic violence. This role focuses on helping participants achieve stability, wellness, and self-sufficiency.
What You'll Do:
- Conduct assessments, intake, and ongoing case management
- Develop and support individualized service plans with participant input
- Connect clients to community resources (housing, employment, healthcare, legal, etc.)
- Collaborate with external providers and maintain compliant documentation
- Track outcomes, maintain case files, and support program reporting
What We're Looking For:
- Bachelor's degree in a related field 1 year relevant experience
- Strong skills in assessment, advocacy, and relationship-building
- Experience working with individuals experiencing homelessness or behavioral health challenges preferred
- Highly organized, detail-oriented, and able to manage multiple priorities
- Valid NYS driver's license
