POSITION SUMMARY:
Under the supervisory direction of the Chief Operating Officer (COO). The Community Services Coordinator is responsible for developing, planning, and managing OCHC’s community-based outreach programs aimed at patient and community outreach efforts, marketing, the promotion of health, and assists executive leadership with fundraising outreach and donor acquisition efforts. The CSC identifies and manages OCHC’s community engagement programs from planning and design through implementation, execution, and evaluation.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Manages specific community health programs from planning and design through implementation, execution, and evaluation.
- Identifies community health programs related to grant funding and assists with developing grant proposals.
- Leads OCHCs efforts in the development of community marketing material, literature, and other activities promoting the value and services of OCHC.
- Manages and supervises OCHCs MNSure Navigator program. Coordinates with RCM Director, Front Desk Manager, and other to ensure program functionality.
- Provides direction for health equity efforts in the department and community, including health equity integration into department programs, developing community partnerships to enhance health equity, and directing community health workers on resource development and promotion.
- Attends and represents OCHC in various community meetings with public agencies, state advisory committees, and professional associations.
- Appropriately disseminates information relative to health promotion, health equity, and community outreach and engagement related topics to executive leadership team.
- Creates and implements strategies focused on advancing health equity by identifying and eliminating patient barriers to equity and improving outcomes for all.
- Researches and analyzes member or community needs to determine outreach program efforts, direction, and goals.
- Promotes the programs and services of OCHC to community groups.
- Supervises, and evaluates assigned employees; ensures timely completion of employee performance plans and appraisals.
- Identifies and addresses employee performance and/or behavior problems in a timely manner and disciplines employees as necessary in accordance with OCHC policy.
- Prepares and maintains community participant records and program reports.
- Performs miscellaneous job-related duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
Baccalaureate degree in business, public or health administration or closely related field required.
Previous experience with underserved community health programs.
Demonstrated experience developing and operationalizing strategies that have taken a program or organization to the next stage of growth.
- Familiarity with and sensitivity to the economic, cultural, and linguistic barriers, problems, and other special needs and characteristics associated with the delivery of health care to special and vulnerable populations.
Strong analytical, problem-solving, and decision-making skills.
PREFERRED QUALIFICATIONS
Experience with the MNsure Navigator program and/or Community Health Worker programs preferred.
- Experience working with patients with low-income and other social determinant of health barriers to care.
- Management experience in an FQHC/community health center.
- Ability to interpret, analyze, formulate plans, organize, and implement organizational decisions.
- Strong interpersonal, communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
- Ability to develop, plan, and implement short- and long-range goals.
- Knowledge of organizational structure, workflow, and operating procedures.
- Ability to supervise and train assigned staff including organizing, prioritizing, and scheduling work assignments.
- Ability to foster a cooperative environment.
WORKING CONDITIONS & PHYSICAL REQUIREMENTS:
Primarily office-based with frequent interaction in clinical and community settings.
Frequent travel required for community events and activities, meetings, and conferences.
- Ability to work in a fast-paced, high-pressure environment.
