The Wound Care Nurse (LPN) is responsible for overseeing skin integrity, wound assessment, treatment, documentation, and discharge planning. This role plays a vital part in improving resident outcomes and maintaining regulatory compliance.
Key Responsibilities
- Perform comprehensive skin and wound assessments
- Obtain and implement physician treatment orders
- Administer wound treatments and document accurately
- Monitor wound healing and recommend treatment changes
- Track pressure ulcers, measurements, and healing progress
- Complete Braden risk assessments and preventive interventions
- Educate nursing staff on wound care best practices and documentation
- Coordinate wound care supplies with central supply
- Assist DON and Medical Director with pressure ulcer reviews
- Participate in QAPI reporting and quality improvement initiatives
- Support discharge planning related to wound care
- Stay current on state/federal regulations and wound care standards
- Graduate of an approved LPN program
- Active Missouri LPN license
- Minimum 1 year wound care experience in long-term care
- 2 years SNF experience preferred
- Current BLS/CPR certification
