The Wound Nurse is responsible for assessing, treating, and monitoring residents with skin conditions and wounds while ensuring the highest standards of care and regulatory compliance. This role works closely with the Director of Nursing, physicians, and interdisciplinary care teams to improve outcomes and maintain resident skin integrity.
Key Responsibilities
- Perform skin assessments for residents and identify wound care needs.
- Obtain and implement physician treatment orders for wound care.
- Provide and document wound treatments and dressing changes according to care plans.
- Monitor and evaluate wound healing progress and recommend treatment adjustments.
- Maintain accurate documentation of pressure ulcers and skin alterations.
- Conduct and record weekly wound assessments and measurements.
- Track and calculate pressure ulcer rates and prevention metrics.
- Ensure completion of Braden Risk Assessments and implementation of prevention protocols.
- Educate nursing staff on wound care protocols, documentation, and prevention practices.
- Assist with Quality Assurance & Performance Improvement (QAPI) initiatives related to skin integrity.
- Collaborate with the Director of Nursing and Medical Director for wound case reviews.
- Ensure adequate wound care supplies and coordinate ordering when necessary.
- Stay current on state and federal regulations and best practices in wound care.
- Registered Nurse / Licensed Practical Nurse (LPN) in the state of practice
- Current BLS/CPR certification
- 1 year experience providing wound care or specialized treatments in long-term care
- 2 years nursing experience in a Skilled Nursing Facility preferred
- Strong documentation and assessment skills
- Knowledge of pressure ulcer prevention and wound management
- Experience with pressure injury staging and wound measurement
- Familiarity with EMR documentation
- Experience participating in QAPI programs
- Strong communication and teamwork skills
