Qualifications
- Bachelor’s degree in Social Work or related field (Psychology, Sociology, Special Education, or Rehabilitation Counseling)
- Minimum of 1 year supervised social work experience in a healthcare setting with elderly populations
- Skilled Nursing Facility (SNF) experience preferred
- Complete psychosocial assessments and develop individualized care plans
- Maintain accurate, timely documentation to meet state and federal guidelines
- Provide emotional support and crisis intervention for residents and families
- Coordinate discharge planning and community referrals
- Advocate for residents’ rights and collaborate with the care team
- Educate staff and ensure compliance/survey readiness
