Title: LICENSED VOCATIONAL NURSE (LVN)

General Statement Of Function

The Licensed Vocational Nurse (LVN) plays a crucial role in promoting the health and well-being of participants by providing comprehensive training, monitoring, and person-centered care. This position is responsible for implementing healthcare plans, conducting training sessions, and closely monitoring health conditions to ensure participants achieve and maintain optimal health. The LVN serves as a key communicator, bridging the gap between staff, medical professionals, and care providers to ensure cohesive and effective care. This role also fulfils the Infection Preventionist, ensuring strict adherence to infection control protocols to safeguard the health of all participants.

Essential Functions

Training and Education

  • Provide monthly training to staff on healthcare conditions, signs and systems of illness prevention, lifting, and other assigned in-service trainings.
  • Provide medication training to new hires within the first 30 days of hire.
  • Conduct annual and/or remedial medication training for current staff.
  • Provide the Infection Control Plan and Emergency Infection Control Plans’ required trainings.
  • Track completed and upcoming trainings as required.

Healthcare Plan Management

  • Provide input in the development of restricted healthcare and care plans ensuring compliance with Title 22 regulations.
  • Monitor the implementation and completion of care documentation (VNS, bowel and bladder, etc.) at each program regularly.
  • Conduct annual restricted healthcare and care plan audits to ensure they are current.
  • Review existing restricted healthcare and care plans as needed.
  • Maintain a tracking log to ensure restricted healthcare and care plans are current.

Communication and Coordination

  • Communicate with staff and managers regarding care concerns of participants.
  • Respond to requests from staff to assess and evaluate participant care concerns.
  • Facilitate communication with other medical professionals when necessary.
  • Foster a supportive and compassionate environment for participants, families, care providers, staff, and visitors.
  • Act as liaison with residential facilities and families on participant care and health conditions and/or concerns.
  • Attend staff team meetings to provide care plan updates, introduce new practices, and/or preventive and healthy habits for participants.

Infection Control

  • Develop and lead the organization’s Health and Wellness Program.
  • Act as the designated Infection Preventionist for all licensed programs, ensuring adherence to infection control protocols and guidelines.
  • Regularly monitor and audit infection control binder within the programs to ensure compliance with established protocols at least annually.
  • Collaborate with managers to ensure the availability and proper use of infection control supplies, such as PPE, disinfectants, and hand sanitizers.
  • Create monthly health and wellness videos that promote infection prevention and healthy practices.

Documentation and Reporting

  • Collaborate with Registered Nurse, Program Manager, and Director of Licensed Programs to develop forms, procedures, protocols, and other assigned documentation to support participants and/or staff in providing optimal care support.
  • Ensure accurate and up-to-date documentation of all care plans, assessments, and evaluations for participants.
  • Document and report any incidents, accidents, or unusual occurrences involving participants, following organizational protocols.
  • Maintain records to demonstrate compliance with Title 22 regulations and other relevant care standards.
  • Prepare and organize documentation for internal and external audits, ensuring all records are complete and accessible.
  • Document detailed progress notes on the health status and interventions provided to participants, ensuring continuity of care.
  • Maintain comprehensive records of all training sessions conducted, including attendance, topics covered, and completion status.
  • Retain detailed logs of communications with staff, program supervisors, medical professionals, residential facilities, and families.
  • Document any updates or changes to participant care needs, ensuring all staff are informed and trained on new protocols.

Qualifications

  • Licensed Vocational Nurse (LVN), with current CA license and knowledge of Nursing Scope of Practice.
  • Preferred experience working with individuals with intellectual and developmental disabilities (IDD) or in a similar caregiving, nursing, behavioral, or human services role.
  • Strong organizational, communication, and leadership skills.
  • Ability to provide comprehensive staff training and coaching.
  • Ability to handle emergencies and make quick decisions.
  • Ability to pass a Department of Justice (DOJ) background check and fingerprint clearance, complete TB test, pre-employment physical, and provide proof of required immunizations (as applicable).
  • Valid CA driver’s license, proof of current vehicle insurance, and must have a clear driving record with three (3) years of driving experience.
  • Reliable transportation to travel and attend off-site meetings.
  • Must be able to work in outdoor environments, which can include, but is not limited to allergens, excessive heat, excessive cold, excessive humidity, dampness or chilling, excessive noise and/or slippery and uneven walking surfaces and indoor warehouse-type settings that may lack heating, air conditioning, and have concrete flooring.
  • CPR, First Aid, and Crisis Intervention (e.g., CPI certifications — or ability to obtain upon hire.

Preferred Qualifications

  • Must possess and demonstrate an advanced level of proficiency in Microsoft Office, including Word, Excel and Outlook.
  • Bilingual (Spanish, ASL, etc.) is a plus.

Work Environment Requirements

  • Able to work in both indoor and community-based environments.
  • Able to lift up to 25 pounds by self and up to 150 pounds with one other person.
  • Assist with physical transfers, if necessary.
  • Able to push a wheelchair with a seated participant up to one (1) mile.
  • Standing, walking, sitting, pushing, bending and pulling required.
  • Squatting and repetitive motion may be necessary.
  • Use chemicals in accordance with manufacturer’s labeled instructions.
  • Adhere to the proper care and responsible use of Oparc issued property, tools, and equipment.

Location: Oparc – ADC Monte Vista (assigned worksite may vary)

Reports To: Program Manager

Schedule: Monday through Friday, 7:30 am – 4:30 pm, includes an ½ hour unpaid lunch. Hours will vary based on the program needs.
