Case Manager – Home Health Agency
We are looking for a compassionate, organized Case Manager to support our clients and care team. This role involves visiting clients in their homes, ensuring their care plans are followed, and helping coordinate services that keep them safe and supported.
Responsibilities
- Conduct in‑home client visits to assess needs and monitor well‑being
- Develop, update, and oversee individualized care plans
- Communicate regularly with clients, families, caregivers, and the office team
- Ensure services are delivered according to agency standards and regulations
- Document visits, assessments, and changes in condition
- Support caregivers with guidance, updates, and care‑related questions
- Report concerns promptly and help resolve issues as they arise
Requirements
- Bachelor’s degree required
- Experience in home health, human services, or a related field preferred
- Strong communication, organization, and problem‑solving skills
- Ability to travel to clients’ homes reliably
- Compassionate, professional, and comfortable working independently
- Must meet agency compliance standards and pass background checks
