Responsibilities:
Provide direct care to patients, including assistance with daily living activities, medication management, and vital sign monitoring
Work closely with our skilled nursing team to develop and implement individualized care plans
Maintain accurate and up-to-date records of patient care and progress
Communicate effectively with patients, families, and healthcare professionals to ensure seamless coordination of care
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
